In September of 2004, the MOCHA Project was awarded a Health Resources and Services Administration (HRSA) Grant under the Special Projects of National Significance (SPNS) Initiative.  The grant is a five year demonstration model that will fund the MOCHA Collaborative to Engage HIV Seropositive Young MCSM.  The collaborative, which has been renamed Project YEAH (Youth Empowerment Around HIV), seeks to identify and engage HIV seropositive and high risk HIV seronegative young MCSM between the ages of 13-24 in the Rochester community.

  • Referrals for STD Related Medical Services
  • Free Condoms
  • One on One Counseling and Referrals
  • Group Level Education
  • Free confidential HIV Testing

Care Collaborative

In order to establish effective youth-oriented medical and ancillary care services, Project YEAH initiated a care network, named the Care Collaborative.  This collaborative consists of a dozen agencies in Rochester, NY.  The Care Collaborative of the Project YEAH seeks to engage young men of color who have sex with men around issues of HIV education, prevention, advocacy and medical services.  The Project YEAH Care Collaborative will identify community resources and provide enrolled clients with the appropriate medical, mental health, vocational and religious support services.  By forging partnerships with local HIV/AIDS service providers, LGBTQ organizations, research and medical institutions, the MOCHA Project hopes to greatly enhance our present linkages while cultivating new collaborations.  In so doing, we wish to close the identified service needs gaps by establishing an effective collaborative to increase testing and enrollment of the target population in enhanced HIV care services.
