Client Services Navigation

The Client Services Navigation Program at MOCHA Project is an initiative which is dedicated to the providing of resource linkage, advocacy, support, education, moitoring, and referral services in an effort to enhance the quality of life among HIV + men of color and specializing in the Men who have sex with men (MSM), gay, Bisexual, and transgender communitites.

The Navigation Services include the following:

  • Intake Services - to determine what in the full array of MOCHA services are appropriate.
  • Assessments - tools to assess consumer need for resources like COBRA Case Management programs, Medical providers, Housing, Financial resources, Emergency assistance and supportive services.
  • Referrals - allow access to all available resources and hands on assistance with obtaining needed resources.
  • Follow Up and Advocacy - ensures linkage and follow through so that the consumer has accessed needed service by monitoring and follow up with providers and the consumer.
  • Education - to reduce the risk and harm to HIV+ individuals.
  • Support Groups - For mem who are HIV+ and groups for Men who are HIV+ and struggling with drug or alcohol additions.
  • Transportation Services - provided through an agreement with the American Red Cross to ensure that MOCHA consumers access to medical appointments, case management programs, support groups and nutritional programs.
